For Veterinarians, this is for your use to track your c.e. completed in 2012. You may click on this pdf link  and print this out. You can use this to enter c.e. as completed throughout the year and submit the table with your 2013 renewal form.

Unfortunately, we can no longer allow submission of pages from programs. Too many lists submitted from programs were hard to read or on pages too dark to ascertain what was written. Also, some thought they could leave it to this office to total the hours as long as they highlighted or circled the title.
